IN-HOME NURSING CARE

At Olidia Care, we specialize in providing care for technology dependent clients with special physical, medical, or emotional challenges. Our clients are assigned case managers who conduct regular monthly assessments at each client's home, review the care that is being provided and thoroughly evaluate the level of care to determine if the services being provided are consistent with the client's personal care needs.

RESIDENTIAL CARE

We provide Residential Home Care to clients who don’t have the infrastructure to receive complex nursing care in the comfort of their private home. The focus is on quality of life, independence and active participation in the community.

ATTENDANT SERVICES

Olidia Care can provide personal care services in the comfort and convenience of your home. These non-medical, non-skilled in-home attendant services include providing assistance with bathing, meal preparation, transfer and mobility.

SKILLED NURSING CARE

Olidia Care provides intermittent skilled nursing services that enable clients with complex medical conditions to receive the specialized care they need right in the comfort of their own homes.
Olidia Skilled Nursing Visits are intermittent nursing visits ordered by a physician and provided by a Registered Nurse (RN) or a Licensed Practical Nurse (LPN). At Olidia Care, our qualified professionals provide the highest level of technical expertise, competence and compassion to ensure the speedy recovery of our clients.

HOME INFUSION

Home infusion therapy is the administration of intravenous (through a vein) medication, such as antibiotics, nutrition, steroids (and many others) to patients in their own homes or in independent, private infusion suites. Small, portable pumps which allow medication to be administered to the patient through an IV line make it possible for many to go about their normal daily activities while receiving their medicine.

DURABLE MEDICAL EQUIPMENT DME

Olidia Medical is a full-service DME provider servicing all of your home medical equipment, supply, and enteral nutrition needs. At Olidia Medical, our mission is to enhance the quality of life of our clients by providing excellent with unmatched service.
We work with a wide range of patients to offer high-quality home medical equipment and respiratory supplies. We work with patients, caregivers, physicians, and referral sources across the Greater Twin Cities Metropolitan area and beyond.

PHARMACY SERVICES

Olidia Pharmacy provides a complete package of pharmacy services to Home Health, Assisted Living and Long-Term Care facilities. Our service includes dispensing intravenous drugs and non-sterile pharmaceutical compounding. We work to provide pharmacy services accurately, efficiently, and cost-effectively. As we work to provide the best pharmacy care and customer service to our patients, our medication delivery schedules are tailored to customer needs.

TRANSPORTATION SERVICES

Olidia Transit provides professional door-to-door non-emergency medical transportation for elderly and disabled persons. We offer wheelchair and stretcher transportation to and from Adult Day Services, Outpatient Surgeries, Hospital Discharges, Dialysis Appointments, Rehabilitation Centers, Mental Health Visits, Outpatient Hospital, Special education Transportation, Long Distance Rides, Leisure Rides, and Medical Appointments.

CARE MANAGEMENT

Olidia Care provides a variety of high-quality services including case management, advocacy and relocation services. We use a client-centered approach in addressing the physical, intellectual, social, and emotional well-being of the people we serve. Olidia Pathways professionals provide personalized support to aging individuals and their families as they manage the many challenges faced during the aging process.

RESPITE CARE

Respite Care is designed to relieve the family caregiver from the constant burden of caring for a chronically ill or disabled Veteran at home. Services can include in-home care, a short stay in an institutional setting or adult day health care.

HOSPICE CARE

At Olidia Hospice, we offer a variety of unique family-based hospice services for veterans with a serious, life-limiting illness, as well as services for loved ones.
Veterans living with end-stage illnesses, and their families, deserve dependable care that caters to their individual needs. Our multidisciplinary team understands the unique needs of veterans. They are familiar with military culture and traumas.

    We assess the patient and work with the patient physician and other stakeholders in the patient care to design a comprehensive plan of care.

  • Team Building

    Our Care Managers assign the most appropriate caregivers who match the skill level, location and personality of your child and your family.

  • Admission

    Stakeholders attend a final multidisciplinary Discharge Care Conference and we admit the patient to our care.

  • Coordination of Care

    We work with our network of multidisciplinary programs and providers to coordinate the care of our patients.
