Chronic Obstructive Pulmonary Disease (COPD)

As the third-leading cause of death in the United States, COPD affects an estimated 15 million Americans, and as many as 12 million more suffer from the disease but are not yet diagnosed.

COPD encompasses a variety of respiratory illnesses including emphysema, chronic bronchitis and refractory asthma.     Stakeholders attend a final multidisciplinary Discharge Care Conference and we admit the patient to our care.
