Olidia Care is a Minnesota based Home Health Care Agency. We provide nursing services to medically complex and technology dependent patients of all ages. We combine our extensive experience in complex nursing care with our compassion to achieve clinical excellence.

At Olidia Care most of our clients receive extended hours of nursing care or skilled nursing care in the comfort of their home. We also provide Residential Complex care in a Vent Home

IN-HOME CARE

At Olidia Care we provide complex nursing care in the comfort of the client home. for complex care.

RESIDENTIAL CARE

We provide Residential Home Care to clients who don’t have the infrastructure to receive complex nursing care in the comfort of their private home.

 

PEDIATRIC HOME CARE

At Olidia Care, we believe that children are not small adults. We provide nursing services to medically complex pediatric patients.

SKILLED NURSING CARE

Olidia Skilled Nursing Visits are intermittent nursing visits ordered by a physician, and provided by a Registered Nurse or Licensed Practical Nurse.

Our Electronic Medical Record System (Olidia IT) is custom designed and developed to meet the individual needs of our clients.

In 2010, Olidia Care became one of the first home health care providers in the State of Minnesota to Go Green and paperless in its operations.

OUR LINE OF CASE MANAGEMENT SERVICES

At Olidia Pathways, we provide excellent assistance moving from a healthcare institution to the community or from one community setting to another.
Our customized supportive programs are designed to promote healthy independent living skills in the community.

ASSESSMENT

We perform comprehensive assessments which include the client's medical history, an assessment of cognitive abilities, functional and emotional status, assessment of strengths, living environment, finances, and existing social support.

COORDINATION OF CARE

We provide coordination of services when multiple disciplines are involved.
Coordination of services may include home health agencies, homemaker and companion services, adult day care, special transportation or assistive technology.

EDUCATION AND COUNSELING

Our Care Managers educate family on self-directed supports and provide valuable information to make good decisions
They also educate seniors and families on the benefits available to them including community, state, and federal programs.

ADVOCACY

Acting as a Health Care surrogate for clients without loved ones available or in-town, we advocate with health care and service providers to ensure that the client understands and approves the care or treatment provided.

RELOCATION

We assist our clients in the relocation process between healthcare facilities or from a facility to the community. We make the necessary arrangements for facility admissions, transfers, and discharges. We research, evaluate, and arrange for senior living options.

FAMILY LIAISON AND PROBLEM SOLVING

As an Objective Third Party, we assist families in the thoughtful decision-making process, we facilitate family meetings regarding care, enabling families to reach consensus and care solutions, even with strained and difficult family dynamics.

FISCAL RESPONSIBILITY

We are fiscally responsible and make every effort to conserve the resources of our clients. We will explain in detail the costs of service to our patients. There are no hidden costs. We are strong advocates and reassuring advocates for our clients. We will always place your interests over ours or any other parties.

HOME CARE SUPPORT

For our clients, we coordinate the access, implementation, and coordination of services in a home setting. We arrange for needed in-home services, service modifications and needed equipment. We oversee and provide support and coaching for home and family caregivers to ensure consistency and quality of care.

MEDICAL EMERGENCIES

If family members are not immediately available, seniors need a responsible local person who can respond quickly to medical and household emergencies. We also help our clients develop and review a personal emergency response system.

INSURANCE SUPPORT

We help and assist our clients as they navigate the complex healthcare and insurance maze. We negotiate with insurance companies for needed eligible services.

PROFESSIONALISM

We are fully qualified and licensed in our profession to provide the services we undertake. We continue to improve our skills and knowledge by participating in professional development programs.

At home the patient gets a one-on-one attention by a highly trained caregiver. She would not receive this level of care as a resident in a long term care facility where an equally skilled caregiver can have up to a dozen patients.
