At Olidia Care, we specialize in providing care for technology dependent clients with special physical, medical, or emotional challenges.

Our clients are assigned case managers who conduct regular monthly assessments at each client's home, review the care that is being provided and thoroughly evaluate the level of care to determine if the services being provided are consistent with the client's personal care needs..

 
 

Care Management

Our Care Managers are available or on call 24 hours a day to make sure that our home healthcare professionals, clients and families have the support they need to deliver or receive the best possible quality of care.

Our care managers perform comprehensive patient assessments and assist in promoting a safe and efficient environment of care. They offer support and education to facilitate the return to optimal physical and mental health of your loved one.

Admission Process

We are committed to providing a level of service that meets or exceeds home care industry’s highest standards.

  • Assessment

    We assess the patient and work with the patient physician and other stakeholders in the patient care to design a comprehensive plan of care.

  • Team Building

    Our Care Managers assign the most appropriate caregivers who match the skill level, location and personality of your child and your family.

  • Admission

    Stakeholders attend a final multidisciplinary Discharge Care Conference and we admit the patient to our care.

  • Coordination of Care

    We work with our network of multidisciplinary programs and providers to coordinate the care of our patients.
