CARE MANAGEMENT

Our Care Managers work closely with your family to build confidence, promote family involvement, improves the quality of live of our patients and fosters independence.

Our case managers are all licensed Registered Nurses. The case manager will come to your home to thoroughly discuss your needs and help you arrange the best service possible.

Your case manager will visit you periodically but is also available by phone. The case manager also serves as supervisor for a group of home care aides. If ever there is a question or concern about your service, the case manager is the person to call!

At times, the case manager may even suggest other community resources that may supplement our home care service.

In our Residential Care Programs, nursing care is only provided by Olidia Care team of highly skilled professionals. We need a minimum of 5 business days to complete the transfer and admission process.
