The Olidia Health Group is the widest network of home care services in the State of Minnesota. It includes Home Health, Assisted Living, Pharmacy, Durable Medical Equipment, Infusion Therapy, Hospice, Stretcher Medical Transit Service and Electronic Medical Record Systems.
We currently have opening for Licensed Practical Nurses (LPN's) and Registered Nurses (RN's) in Rochester.
DUTIES AND RESPONSIBILITIES
Here are your main duties as a home care nurse:
1. Assess patient's physical functions and psychosocial needs upon admission to the home care programs.
2. Coordinate home care nursing with other members of the health care team
3. Set priorities of patient's needs and provides care accordingly.
4. Identify patient problems, symptoms and behavioral changes.
5. Execute the patient's nursing care plan that integrates the medical plan of care, nursing diagnosis, realistic goals and established discharge criteria.
6. Document the patient's condition accurately and concisely, using correct charting format: notifies the physician of condition changes.
7. Carry out physician's orders in accordance with the agency policies.
8. Assess patient and family about disease processing in accordance with their degree of acceptance.
9. Teach the patient and family about disease processing in accordance with their degree of acceptance.
10. Supervise and evaluate the performance of the Home Health Aide as needed: at least every sixty days: documents accordingly
11. Practice nursing within the legal limits of the Nurse Practice Act and understand and anticipate legal consequences of nursing action.
12. Maintain current knowledge in nursing procedures.
13. Seek educational opportunities to increase clinical competency.
14. Insure privacy for the patient and confidentiality of condition and personal information.
15. Participate in care conferences, staff meetings and committees.
16. Participate in teaching and orientation of all personnel.
17. Cooperate and acts as liaison with other agencies providing nursing or related services to implement a comprehensive care plan.
18. Participate with other Staff Nurses in taking weekend, holiday and night calls on a rotation basis as scheduled.
19. Participate in the planning, operation and over-all evaluation of the nursing service.
20. Participate in preventative and rehabilitative nursing procedures as appropriate for the patient's care and safety, without infringing on the patient's rights.
21. Provide appropriate documentation of activities and interventions according to agency standard.
